Social Worker Assistant
Job Summary:

Responsibilities:
Assist social workers in conducting initial and on-going assessments of patients and residents to identify their physical, emotional, and social needs
Develop and implement individualized treatment plans to meet the unique needs of each patient and resident
Provide supportive counseling and therapy to patients and residents, as well as their families and caregivers
Collaborate with healthcare teams to ensure comprehensive care and support
Maintain accurate and detailed records of patient and resident care
Participate in interdisciplinary team meetings to discuss patient and resident progress and develop strategies for improved outcomes

Requirements:
BACHELORS degree in Social Work or a related field
2 years of experience in social work or a related field
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Proficiency in Electronic Medical Records (EMRs)
